Social Service Coordinator

Foundation of Resident Care

Purpose

Using a person-centered, compassionate approach, the Social Service Coordinator ensures residents of St. Ann’s are given care of the highest quality, encompassing all elements of physical, emotional, and spiritual needs while helping residents live their lives with dignity and to the fullest.

Job Summary:

The primary purpose of your job position is to plan, organize, develop, and coordinate the operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Nursing/ Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Advocates

  • Assist the Residents in Adjusting to the facility and promote a positive environment for the Continuity of Relationship with Family and Community
  • Assist Residents, and Families to Utilize the Community Resources when Not Provided Directly by the Corporation
  • Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.
  • Assess and Evaluate Each Resident's Psychosocial Needs and Develop Goals for Providing the Necessary Service and Take Part in Admission Process as Needed

Collaborative

  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Serve on, participate in, and attend various committees of the facility as required, and as appointed by the Administrator.
  • Participate in daily nursing report along with daily electronic documentation
  • Meet with administration, medical and nursing staff, as well as other related departments in planning social service programs and activities.
  • Assist Director of Nursing/Administrator in abuse/neglect/misappropriation investigations.
  • Consult with/support Assisted Living Manager with any psychosocial issues that may arise
  • Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken

Quality-driven

  • Assist the Quality Assurance committee in developing and implementing appropriate plans of action to correct identified deficiencies.
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator.
  • Incorporate the Social Service Goals in the Resident's Plan of Care, Attend Care Planning Conferences and Family Council.
  • Ensure that social services components of the MDS are completed and signed on a timely basis.
  • Assist in the development, administering, and coordinating of department policies and procedures.
  • Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed.

Our Social Service Coordinator have

  • LBSW from an approved school of Social Work
  • Minimum of 2 years of experience in supervisory capacity in a hospital, nursing care facility, or other related medical facility
  • Must be a member in good standing in the National Association of Social Workers and Academy of Certified Social Workers, Inc.
  • Demonstrated commitment to care excellence.
  • Demonstrated commitment to a person-centered approach.
  • Demonstrated compassion.
  • Strong interpersonal and communication skills to work effectively with residents and team members.
  • Knowledge of electronic medical record software or ability to quickly learn software.
  • Mental, emotional, and physical strength and stamina to perform the work required above with or without reasonable accommodation.
  • English language fluently in order to communicate with patients, families, care team, and other team members in written and verbal form.
  • Passed a drug screen, 25lb lift test, and general pre-employment physical.
  • Maintenance of ongoing required trainings.

Job Type: Full-time

Pay: $50,000.00 - $52,000.00 per year
